 Health Center Quality Leader – Silver


Frontera Healthcare Network received the Health Center Quality Leader – Silver designation from HRSA, an honor awarded to health centers that demonstrate exceptional clinical quality and patient outcomes. Ranking among the top 20% of health centers nationwide, this recognition reflects strong performance in areas that directly impact our patients, including preventive care, chronic disease management, screenings, and overall health outcomes. This award represents Frontera’s continued commitment to raising the standard of care and providing high-quality healthcare to the rural communities we serve.


National Quality Leader – Behavioral Health

For the second year in a row, Frontera Healthcare Network was recognized by HRSA as a National Quality Leader in Behavioral Health, an achievement that reflects exceptional performance in caring for patients’ mental and behavioral health needs. This recognition considers measures such as identifying depression early, ensuring patients receive appropriate follow-up care, supporting patients in achieving improvement from depression, and screening for substance use concerns. For our patients, it represents a commitment to making behavioral healthcare an important and integrated part of whole-person care.

 
 
National Quality Leader – Diabetes Health
 
Frontera Healthcare Network was recognized as a National Quality Leader in Diabetes Health, highlighting exceptional performance in helping patients prevent, manage, and improve conditions related to diabetes and overall health. This recognition considers measures such as diabetes control, healthy weight assessment, and providing patients with appropriate nutrition, physical activity, and follow-up support. For our patients, it reflects the work happening every day to help people better understand their health, manage chronic conditions, and reduce the risk of serious complications.

 
 
Improving Health Care Access
 

Frontera received HRSA’s Improving Health Care Access recognition for our efforts to connect more patients with the healthcare and support services they need. This designation reflects both growth in access to care and a continued focus on quality—ensuring that as we reach more people, we remain committed to providing care that makes a meaningful difference in their health.


Advancing Health Information Technology (HIT) for Quality

Frontera received the Advancing Health Information Technology for Quality recognition for using technology to make healthcare more accessible, connected, and responsive to our patients’ needs. This includes tools such as telehealth, secure exchange of health information, and technology that helps patients stay engaged in their care—supporting better communication and continuity across the healthcare experience.


Preventive Health
 
Frontera received the Preventive Health recognition for strong performance in helping patients stay healthy through recommended screenings, early detection, and preventive care. From cancer screenings and healthy weight assessments to depression and tobacco-use screening, these measures represent opportunities to identify health concerns earlier and give patients the tools and support they need to protect their health.
 
NACHC Gold Advocacy Center of Excellence
 

Frontera Healthcare Network has once again earned Gold Advocacy Center of Excellence (ACE) status from the National Association of Community Health Centers, the highest level of recognition within the ACE program. This designation recognizes health centers that make advocacy a meaningful part of their mission and actively work to ensure the needs of their patients and communities are heard. For the rural communities Frontera serves, that means advocating for the resources, policies, and support necessary to protect and strengthen access to healthcare for years to come.
