History

2002, the leadership of Concho Valley Health Foundation first inquired into the requirements of establishing a community health center for Concho County.

2003, leaders of the Mason County Hospital District board contacted the Texas Association of Community Health Centers and a private health consultant to determine if pursuing Section 330 funding was an option.

2004, the impending closure of Concho County’s only private health clinic prompted the Concho Valley Health Foundation to take specific action to identify a viable long-term solution for the provision of primary care in the community. Late 2004, the Foundation invited representatives of the hospital and civic leaders from Eden, Mason, and Menard counties to a forum to discuss the possibility of collectively seeking a Section 330 CHC grant for the three county region. 

2005, a series of meeting were held to address various facets of the proposed plan, and Concho County Hospital agreed to sponsor a Rural Health Network grant request to the Office of Rural Community Affairs of Texas. This grant would form the basis for further collaboration and integration of individual county efforts. This grant was awarded in March of 2005.

December 2006, Frontera Healthcare Network submitted a New Access Point (NAP) application to HRSA

November 1, 2007 FHN began offering medical services at medical facilities located in Eden and Menard.

November 2008 FHN began offering medical services in Mason and dental services in Menard. Mental health services are available at all three medical care facility locations at specified times during the week.

2009, FHN applied for and received a CIP grant to complete the structure that the CCH Hospital owned.  Frontera moved into the building in July of 2010.

2010, FHN began it’s first 5 year grant cycle.

2011 FHN wrote an expansion grant to expand to Kimble and McCulloch counties.  The grant wasn’t awarded until June of 2012. 

October of 2012, Opened sites in Junction and Brady, expanding Frontera’s footprint to 5 counties and 6 sites.

2012, we contracted with CCH pharmacy to provide 340b services for all of FHN.  The partnership has worked well for our communities. 

2016, FHN began a collaborative Behavioral Health project with Esperanza Medical and Dental Clinics and West Texas Counseling and Guidance to provide counseling and Tele-Health services. Grant support from Methodist Healthcare Ministries supplied a Behavioral Health Coordinator to integrate BH services with Primary Care.

2017, FHN purchased Dr. Dalyn J. Johnson’s dental practice and began operating both of his clinics in Eden and Brady in March. 

2018, Launched Chronic Care Case Management. Achieved Patient Centered Medical Home Accreditation. Methodist grant provided digital x-ray services in both dental clinics.  Dental clinics converted to electronic dental records.  New X-ray equipment in Mason from a tremendous fundraising project of the community. Began providing Tele-Psych services through a partnership with Methodist Healthcare Ministries.

2019, Hired our first pediatrician and expanded services to include pediatrics at our Mason clinic. FHN also began providing Tele-Medicine services in addition to the Tele-Health services already in place. Hired our second dentist for our dental practice. Integrated Behavioral Health Service grant that allowed for greater integration of behavioral health services with medical services.

2020, Covid pandemic. Fully switched to telehealth in less than two weeks. In December 2020 shut down dental and medical clinics in Eden. Expanded Brady clinic to include administrative offices.

2021, Leaders in Covid 19 vaccination and recovery. Moved to new Junction location in March 2021, opened Healthcenter Pharmacy April 2021, and opened Fredericksburg Clinic in June 2021. Started Rural Health Initiative with TAMU to encourage medical doctors to come practice in rural Texas.

2022, In May FHN responded to Uvalde Shooting to help CHDI set up behavioral health. Change in leadership from Cam to Mikki. Cam was ED from March 2008 to August 2022. Received MHM grant for Social Determinants of Health Coordinator, Patient Assistance Coordinator, and Rural Health Program Coordinator. Launched our LPC-a Training Program.

2023, Launched online bill pay and patient portal. Started Confermed for e-consults with specialists to help patients who cannot get to specialist or who cannot get to specialty for a period of time. Frontera also joined a Clinically Integrated Network and an Accountable Care Organization
